脑梗死慢性期抑郁状态的磁共振弥散张量成像初步研究

A Preliminary Study of Magnetic Resonance Diffusion Tensor Imaging on Depressed State in Chronic Phase of Cerebral Infarction

摘要: 目的 探讨磁共振弥散张量成像(diffusion tensor imaging,DTI)技术在脑梗死慢性期抑郁状态患者脑白质细微结构变化中的价值,从脑白质结构细微变化的角度初步探讨卒中后抑郁(post-strokedepression,PSD)的发病机制。方法 选择首次发病、单侧病灶、既往无抑郁病史及未接受抗抑郁药物治疗的急性脑梗死患者,收集其人口学资料及相关临床指标。在发病90±7 d完成汉密尔顿抑郁量表(Hamilton Rating Scale forDepression,HRSD),并根据HRSD结果分为脑梗死慢性期抑郁状态组(HRSD≥8)和非抑郁状态组(HRSD<8)两组,比较两组脑内解剖结构的磁共振DTI序列各向异性分数(fractional anisotropy,FA)和偏侧化指数(laterality index,LI)的差异。结果 本研究共入组脑梗死慢性期抑郁状态组13例患者,脑梗死慢性期非抑郁状态组15例患者。抑郁状态组患者双侧额中回的白质FA值较非抑郁状态组低(右侧468.77±90.14 vs 565.26±112.33,P =0.02;左侧452.15±50.24 vs 539.16±85.19,P =0.003)。两组间不同解剖部位的双侧FA值以及两组间不同解剖部位的偏侧化指数比较,差异均无显著性。结论 DTI技术可显示脑梗死慢性期抑郁状态患者脑内存在白质纤维束细微结构的改变。脑梗死慢性期抑郁状态患者双侧额中回FA值降低,提示前额叶背外侧神经通路的损害可能构成了脑梗死后发生抑郁状态的神经生物学基础。

Abstract: Objective To study the value of diffusion tensor imaging(DTI) in microstructural changes in brainwhite matter of depressed patients in chronic phase of cerebral infarction and to preliminarily studythe pathogenesis of post-stroke depression(PSD) from the perspective of microstructural changes inbrain white matter.Methods We chose those right-handed patients with first ischemic stroke and infarcts affectedunilateral hemisphere. The patients who are taking antidepressant medication at the time or hada past history of depression were excluded. The demographics and clinical information of allthe patients were collected. At the 90-day follow up, all patients recruited in this part underwentmagnetic resonance imaging(MRI) and Hamilton Rating Scale for Depression(HRSD) andwere separated into two groups according to the score of HRSD:depressed (HRSD≥8) andnon-depressed(HRSD<8) patient groups in chronic phase of cerebral infarction. The fractionalanisotropy(FA) values and laterality index in different regions were measured automatically by theworkstation and compared between the two groups.Results Thirteen patients in depressed state and 15 patients in non-depressed state in chronicphase of cerebral infarction were involved. Depressed patients in chronic phase of cerebralinfarction exhibited significantly decreased FA values in bilateral middle frontal gyrus, comparedwith non-depressed patients(right:468.77±90.14 vs 565.26±112.33, P =0.02; left:452.15±50.24 vs539.16±85.19, P =0.003). No significant differences were observed in the laterality index of allregions between two groups as well as the bilateral FA values of all areas.Conclusion DTI is a sensitive method for detecting the microstructural changes in brain whitematter of depressed patients in chronic phase of cerebral infarction. Decreased FA values inbilateral middle frontal gyrus of depressed patients in chronic phase of cerebral infarction suggestthat damage to the dorsolateral prefrontal circuit might be one of neurobiological mechanism of thedevelopment of depressed state post-stroke.
