    Objective To assess the applying of tracheotomy in early stage in severe stroke patients.Methods We investigated 102 stoke patients who underwent endotracheal intubation in 48 hours after admission retrospectively. All the patients were divided into two groups, early tracheotomy group (n=55) who underwent tracheotomy within 5 days after endotracheal intubation, and delayed tracheotomy group (n=47) who underwent tracheotomy after 5 days. We compared the mortality, dosage of sedative, incidence of hospital acquired pneumonia(HAP), duration of mechanical ventilation and hospitalization in intensive care unit(ICU) of the two groups.Results Early tracheotomy could reduce the duration of mechanical ventilation(178±94 h vs 266±162 h, P=0.03) and hospitalization in ICU(10±5 d vs 13±5 d, P=0.006), and it could also reduce the duration of antibiotic(19±4 d vs 12±4 d, P=0.03) and the dosage of sedative significantly(138.72±38.77 mg vs 186±48 mg, P=0.001). However there was no significant difference in mortality (29.1% vs 36.2%, P=0.45) and incidence of HAP(49.1% vs 63.8%, P=0.13) between the early tracheotomy group and delayed tracheotomy group.Conclusion Severe stroke patients can get some benefit from early tracheotomy.
    Effects of Lipoic Acid on Lipids Accumulation in Vascular Smooth Muscle Cells Induced by ox-LDL

    Objective To investigate the effects of lipoic acid on triglyeride(TG), total cholesterol(TC), and the activities of superoxide dismutase(SOD) and glutathione peroxidase (GSH-PX) in cultured vascular smooth muscle cells(VSMC) induced by ox-LDL.Methods Twenty special pathgon free and male SD rats were sacrificed and isolated their thoracic aorta VSMC. VSMC was divided into five groups: control, ox-LDL, ox-LDL with different lipoic acid (20,80,100&#8197;mg/L) and the control group donot were gave any drugs, and four experiment groups were exposed to ox-LDL and then incubated with different concentrations of lipoic acid respectively. The levels of TG and TC were measured and the activities of SOD and GSH-PX were detected in VSMC.Results It were found that the levels of TG, TC in ox-LDL group was significantly higher than those in the control group(P<0.05), the activities of SOD and GSH-PX were much lower than those in the control group(P<0.05). Lipoic acid could significantly lower the levels of TG, TC(P<0.05) and enhance the levels of SOD and GSH-PX(P<0.05) in VSMC induced by ox-LDL.Conclusion Lipoic acid can inhibit lipid accumulation in VSMC induced by ox-LDL. The mechanisms may be associated with the increase of self-anti-oxidation ability of lipoic acid.
    Risk Factors Analysis in Stroke Patients with Lower Respiratory Tract Infection

    Objective To explore the risk factors and preventive measures for lower respiratory tract infection(LRTI) in stroke patients.Methods A retrospective analysis in 303 patients with stroke was carried out to investigate the incidence rate of LRTI. After one-way analyzing each related factor, the factors with P<0.05 were chosen to make non-conditional Logistic regression analysis.Results The incidence rate of LRTI in stroke patients was 35.97%. Related high risk factors in order were disturbance of consciousness, lesion location, invasive operation, diabetes mellitus, stroke type, and the OR values were 11.938, 2.751, 2.642, 2.381, 1.966 respectively.Conclusion More attention should be paid in nursing care and preventive measures to the stroke patients with high risk factors such as disturbance of consciousness, diabetes mellitus, so as to lower the occurrence of LRIT.

    Low-dose aspirin aspirin exerts anti-thrombotic action by acetylating and inactivating cyclooxygenase-1, preventing the production of thromboxane A2 in platelets, therefore low-dose aspirin is considered as a preventative of ischemic diseases such as myocardial and cerebral infarction. However, many studies have revealed that aspirin has other beneficial actions in addition to its antiplatelet activity. In this review, we discuss the involvement of inflammation in atherosclerosis and how Low-dose aspirin aspirin exerts its anti-inflammation action in atherosclerotic diseases.

    Magnetic resonance diffusion tensor imaging (DTI) is the water diffusion imaging technique developed in recent years, which quantitatively reflects the integrity of white matter fiber tracts by the diffusion of water molecules in human brain. It is mainly used to evaluate the integrity of white matter especially the white matter fiber tracts which influenced by neurologic diseases. So far, DTI is the unique examination which can quantitatively reflect the integrity of white matter fiber tracts by the diffusion of water molecules in human brain without any intrusion. This technique can analyze the diffusion characteristic of the affected tissue and normal tissue quantitatively, exhibit the relationship between the intracerebral affections and the white matter fiber tracts directly. Therefore, it provides more information for diagnosis and prognosis of the diseases. This article reviews the principle of DTI and summarizes the current researches of its common clinical application in cerebrovascular diseases.
