    Objective To evaluate cerebral vasomotor reactivity(CVR) in patients with different types of ischemic stroke and to provide evidence for adjusting optimal blood pressure level in secondary prevention of stroke.Methods We evaluated the breath-holding index(BHI) using the breath-holding index technique during bilateral Transcranial Doppler monitoring of flow velocity in the middle cerebral arteries(MCA) of 70 patients with symptomatic middle cerebral artery atherostenosis(MCAS) and30 patients with lacunar infarction and 36 sex- and age-matched healthy volunteers.Results BHI was significantly lower in patients with symptomatic MCAS in bilateral MCAs than that in the control subjects (0.91±0.43&#8197;vs 1.53±0.80; 1.24±0.42&#8197;vs 1.53±0.80, P<0.01) and BHI in MCA with infarction focus was significantly lower than with no focus(0.91±0.43 vs 1.24±0.42). Compared with controls, BHI in patients with lacunar infarction was significantly lower(1.28±0.86 vs 1.53±0.80, P<0.05). Moreover, BHI decreased progressively with changing of the severity of cerebrovascular stenosis(0.65±0.36 vs 1.41±0.59, 0.65±0.36 vs 1.11±0.42, P<0.05).Conclusion The cerebral vasomotor reactivity is impaired in patients with symptomatic MCAS and lacunar infarction. CVR assessment is helpful to adjust optimal blood pressure level in secondary prevention of stroke.

    Objective To study the apolipoprotein E gene(ApoE) polymorphism in patients with cerebrovascular disease(CVD).Methods The polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) technique were used to detect ApoE genetype in 98 CVD patients(78 cerebral infarction and 20 cerebral hemorrhage) and 90 age-matched controls. We used enzyme chromatometry to detect the level of lipids.Results The frequency of genotype E3/4 was 23.1% and E4 was 14.7% in the cerebral infarction group, significantly higher than those in control group (P<0.05). The frequency of genotype E3/3 was 56.4% and E3 was 75.6% in the cerebral infarction group, which were significantly lower than in the control group(P<0.05). While in cerebral hemorrhage group the ApoE genotype and the frequency of allele had no significant difference when compared with control group(P>0.05). The serum levels of total cholesterol(TC) and low density lipoprotein-cholesterol(LDL-C) were significant difference in different ApoE genotype subgroups(P<0.05).Conclusion ApoE4 is one gene of cerebral infarction while ApoE3 has protection to cerebral infarction. The polymorphisms of apolipoprotein E gene have effect on the serum level of TC and LDL-C.

    Effect of Lipoic Acid on Proliferation of Rat Aorta Smooth Muscle Cells Induced by ox-LDL

    Objective To investigate the influence and the mechanisms of antioxidant lipoic acid on the proliferation of rats aorta vascular smooth muscle cells (VSMC) induced by ox-LDL.Methods The cultured rats aorta VSMC were divided into five groups, untreated control group, ox-LDL group and three groups with different dose of lipoic acid(20mg/L, 80mg/L and 100mg/L). Methyl thiazolyl tetrazolium(MTT) assay and biochemistry detection were used to determine the effect of lipoic acid on anti-proliferation and the levels of malondialdehyde(MDA) and nitric oxide(NO) in the cultured rat aortic VSMC induced by ox-LDL.Results It was found that the proliferation of VSMC and the level of MDA in ox-LDL group were both higher than those in the control group(P<0.01, P<0.05), and the level of NO was lower than that in the control group. Compared with ox-LDL group, three groups with different dose of lipoic acid had lower level of proliferation of VSMC and MDA and higher lever of NO(P<0.05). Furthermore, among the three groups with different dose of lipoic acid there were significantly different in the level of proliferation of VSMC, the level of MDA and NO(P<0.05).Conclusion Lipoic acid can not only inhibit the proliferation of rat aortic VSMC induced by ox-LDL, but also lower the amount of MDA and enhance the amount of the NO in VSMC, that may be the mechanisms of delaying the development of atherosclerosis.

    Objective To evaluate the effect of low molecular weight heparin (LMWH) on transient ischemic attack (TIA).Methods It was a prospective, non-random and control study. Total 102 patients suffered from first onset of TIA were included and divided into two groups: 48 cases in LMWH group and 54 cases in control group. Regulating blood pressure, reducing blood sugar, aspirin 50 mg and Ozagrel 160 mg, once per day were all done to all patients. Five thousand units of LMWH were given to patients in treatment group twice a day for 7 to 10 days. Common situations like age, sex, ischemic type(internal carotid artery system or vertebrobasilar artery system), lasting time, times of onset, risk factors had no significant difference between the two groups.Results The curative effect of TIA in the near future and prognosis after 30 days showed no significant difference between the two groups(p>0.05). There was no significant difference between the two groups in the rate of stroke(including TIA recurrence and cerebral infarction) and myocardial infarction after three months. Two patients had gingival bleeding in treatment group and no patient suffered from bleeding in control group.Conclusion Compared with control group, LMWH shows no superiority in therapy of TIA.
