Abstract:

Objective To investigate the influencing factors of the community residents' willingness to sign up for a family doctor service by implementing a family doctor service contract project in Songshan New District of Jinzhou City. Methods The convenience sampling method was used in this study, a questionnaire survey was conducted on the willingness of signing up for family doctors service among the residents of 11 communities in Lingnan Street, Songshan New District, Jinzhou City from March to June 2017. The content of this survey included the residents’ general situation, health status, the level of understanding this service and the related publicity. Multivariate logistic regression analysis was used to explore the factors affecting the residents’ willingness to sign a contract. Results A total of 2500 questionnaires were sent out in this survey, 2266 valid questionnaires were collected, and the effective response rate was 90.6%. 71.8% (1626/2266) residents were willing to sign the contract. The multivariate analysis results showed that the higher age (OR 2.026, 95%CI 1.561-2.631, P <0.001) and education (OR 1.180, 95%CI 1.062-1.311, P =0.002) and understanding level (OR 1.211, 95%CI 1.065-1.377, P =0.004), as well as the residents who worked in administrative institutions (OR 1.323, 95%CI 1.002-1.576, P =0.047), and received this service publicity (OR 3.026, 95%CI 2.355-4.364, P <0.001), were more willing to sign up for family doctor service.

Conclusions The overall level of the community residents’ willingness to sign up for a family doctor service was higher. The residents who were elder, had higher education and more understanding, worked in administrative institutions and received this service publicity were more willing to sign family doctor service contract.

目的 以锦州市松山新区为试点推行家庭医生签约服务项目，了解社区居民家庭医生签约意愿的影 响因素。 方法 采用便利抽样方法，2017年3-6月对锦州市松山新区凌南街道的11个社区居民进行家庭医生 签约服务意愿问卷调查。调查内容包括居民一般情况、健康状况、对家庭医生签约服务的了解程度等。 采用多因素Logistic回归分析，探讨签约意愿的影响因素。 结果 本次调查发放问卷2500份，有效问卷2266份，有效应答率90.6%，71.8%（1626/2266）的社 区居民有意愿签约家庭医生。多因素分析结果显示，年龄（OR 2.026，95%CI 1.561～2.631，P＜0.001）、 文化程度（OR 1.180，95%CI 1.062～1.311，P =0.002）、了解程度（OR 1.211，95%CI 1.065～1.377， P =0.004）越高，以及在行政事业单位工作（OR 1.323，95%CI 1.002～1.576，P =0.047）和接受过家庭 医生签约服务宣传（OR 3.026，95%CI 2.355～4.364，P＜0.001）的社区居民更有意愿签约家庭医生。 结论 锦州市松山新区社区居民家庭医生签约意愿较高。相较而言，高龄、文化程度高、对签约服 务了解程度高、在行政事业单位工作以及接受过家庭医生签约服务宣传的居民更有意愿签约。
