逆向半回收——改良的solumbra取栓技术

Retrograde Semi-Retrieval Technique——Modified Solumbra Technique for Thrombectomy

摘要:

血管内治疗急性缺血性卒中已经成为一项标准的治疗技术。多重神经血管装置的结合 使用可为技术的改良提供更多的可能性。我们描述了一种改良的取栓技术——使用远端颅内导管 (distal intracranial catheter,DIC)在取栓之前半回收取栓装置。这项技术的成功运用包括回拉DIC使支 架完全释放,前推DIC部分地回收支架,从而使支架在取栓之前形成锥形,更好地嵌合和固定血栓、 缩短取栓行程,同时减少血管内皮损伤。该项技术可作为血栓切除术的一个良好选择,并可提高取 栓效率。

Abstract:

The endovascular treatment of acute ischemic stroke has become a standard therapeutic technique. The combination of multiple neuro-endovascular devices provides more possibilities for the improvement of thrombectomy technique. We will describe a modified retrieval technique by using a distal intracranial catheter (DIC) for the semi-retrieval of stent retriever during the thrombectomy procedure. The retrograde semi-retrieval technique involves completely unfolding of the stent retriever by pulling a Navien catheter and the partial resheath of a retrievable stent forming a tapered configuration by pushing the Navien catheter, which led to a embedding of thrombi into the strut, a securing of the clot within the stent, and a shorter distance for the clot to travel. This technique may lessen the occurrence of thrombus fragmentation and the injury of vessel wall. The retrograde semi-retrieval technique is an optional thrombectomy technique that may improve the efficiency of this procedure in the treatment of acute ischemic stroke.
