大脑半球毛细血管扩张症2例及文献回顾

Abstract: Objective Intracranial capillary telangiectasia (ICT) is a rare disease with few clinical reports. Based on an analysis of two possible clinical cases at our hospital and a review of the literature, we hope to improve awareness of ICT, as well as the ability of diagnosis and treatment.Methods We report on two possible cases of cerebral hemispheric capillary telangiectasia treated at our hospital between 2000 and 2007 and their clinical, imaging, pathology, and postoperative data.Results Both patients were treated surgically with good outcomes. Based on preoperative imaging, intraoperative observations, and postoperative pathology reports, we believe these two cases to be consistent with ICT.Conclusion MRI and CT could play a supportive role in making a differential diagnosis of ICT, but the final diagnosis should be made with the aid of pathological findings. When symptoms and imaging suggest cerebral hemispheric capillary telangiectasia, surgery may be considered as a viable option. However, surgical removal should be complete, and not a partial excision or biopsy.

摘要: 目的 颅内毛细血管扩张症(intracranial capillary telangiectasia,ICT)是一种罕见疾病,临床认识不足。本研究拟通过对临床上2例可疑病例的分析并结合文献回顾,提高对ICT的认识能力及诊治水平。方法 对2000~2007年我院收治的2例可疑大脑半球ICT临床表现、影像学表现、术中所见及术后病理进行分析。结果 2例患者均经手术治疗,术后效果良好。结合术前影像学资料、术中所见及术后病理,符合ICT表现。结论 CT与核磁共振(magnetic resonance imaging,MRI)在ICT 的诊断及鉴别诊断中起辅助作用,最终诊断应结合病理。有症状的大脑半球ICT可考虑手术治疗,应一期全切除,不宜活检及部分切除。
